Several summers ago I visited Camp Pleasant intentional community and organic farm in rural Kentucky. An herbalist living there taught me about Jewelweed which has rescued me from poison ivy numerous times. Jewelweed grows near poison ivy and can be identified easy by the shape of its leaves. Additionally it appears metallic when submerged in water. The stem and leaves contain a fibrous, oozy substance that can be smeared on the skin to counter the affects of poison ivy before or after contact. In my experience no over-the-counter medicine works quite as well.
Here's a video that does a good job of explaining how to identify it.
